CREDIT AUTHORIZATION & CONSENT

We hereby give our consent to have First Allied Mortgage, their assigned Credit Bureau, and their assigned investors, obtain any and all information regarding our employment, checking and/or savings accounts, credit obligations, and any other credit matters, which they may require in connection with my/our application for a loan. This form may be copied or xeroxed, and that copy shall be as effective as this original, which we have signed below.

I certify that I have informed First Allied Mortgage about any other loans including mortgages, auto loans and credit cards being processed at this time. I also agree to promptly notify First Allied should I apply for a loan or credit elsewhere during the loan application process.
