First Allied Mortgage Instructions for Completing Loan Application
1. Required for all loans
Complete and sign 3- or 4-page residential loan application Form 1003 together with the attached credit authorization and fair lending notice. Page 4 of the application is a continuation page in case you need additional space for your assets or liabilities. If you make a mistake while filling out the application cross it out, and make a change. Do NOT use white-out.
If you are salaried: provide the last two years W-2 and one month of pay stubs, OR if you are self-employed: provide the last two years tax returns and a YTD profit and loss statement.
If you own rental property, provide rental agreements and the last two years tax returns.
Provide copy of divorce decree if applicable.
If you are NOT a US citizen provide us with a copy of your green card (front & back), OR if you are NOT a permanent resident provide us with your H-1 or L-1 visa.
2. If you are buying a house or refinancing an existing home
If you wish to speed up the approval process, please also provide the last three months’ bank statements for each bank, stock and mutual fund account.
Provide recent copies of any stock brokerage or IRA/401K accounts that you may have.
3. If you are applying for a home equity loan or a credit line
Please provide a copy of the note on your first mortgage. This is usually to be found in your closing loan documents.
Please provide a copy of your homeowner insurance policy.
Please provide a signed letter explaining what you plan to do with the proceeds.
